About Activate Care: At Activate Care, we’re on a mission to improve health equity and drive improved health outcomes across the country. Our Community Care Record platform, Care Link, enables healthcare and community organizations to coordinate care for populations challenged with health-related social needs (HRSN). Path Assist is our tech-enabled Community Health Worker program for addressing HRSN utilizing an evidence-based, structured intervention. Our goal is simple: address individuals’ unmet HRSNs, increase health confidence, improve self-efficacy, and reduce inappropriate healthcare spend. Role Overview: The Care Coordinator supports individuals and families enrolled in Medicaid by connecting them to vital health and social services. This remote role requires work hours in standard business hours in EST timezone. The Care Coordinator conducts screenings to identify unmet needs, coordinates referrals through established Social Care Networks, and follows up to ensure services are received and goals are met. With a client-centered approach, this position plays a key part in strengthening communities and promoting long-term resilience. Responsibilities: Engage Medicaid-enrolled individuals and families to assess health and social service needs through the HRSN screening within the AHC model. Obtain consent and conduct screenings both virtually and in person across all five NYC boroughs, ensuring accurate and timely data entry into the Social Care Network (SCN) platform. Evaluate eligibility for Enhanced Health-Related Service Needs (EHRSNs) and identify the appropriate level of support and follow-up required. Coordinate referrals to borough-based Social Care Networks (such as SOMOS, Unite Us, or SIPPS) for qualifying members, and guide non-qualifying members to appropriate community-based resources. Track and document all client interactions and service outcomes in accordance with agency and partner requirements, maintaining up-to-date and compliant case records. Follow up with clients to ensure service connection, monitor outcomes, and provide continued support following significant life events or changes in circumstance. Provide technical assistance to internal teams, including Care Navigators and Health Home Care Managers, on SCN platforms and related systems. Collaborate with cross-functional teams and community partners to promote coordinated care, client engagement, and equitable access to resources. Contribute to a client-centered, data-informed care environment that promotes wellbeing, empowerment, and improved health outcomes across the community. Qualifications & Skills: High School Diploma or GED with at least one year of experience in social services, child welfare, or related human services; Bachelor’s degree preferred. Experience providing service coordination, referrals, or linkages to community-based programs. Strong communication, interpersonal, and problem-solving skills with a compassionate, client-centered approach. Excellent time management, organization, and documentation abilities. Proven ability to work both independently and collaboratively within a multidisciplinary team. Comfortable using technology and digital platforms for client documentation and referrals. Flexible, open to feedback, and committed to continuous professional growth. Must be able to travel to Foundling offices as needed and maintain a strong commitment to safety and quality service delivery.
