Location: Remote (Work from Anywhere in the U.S.) Schedule: Full-Time (Monday–Friday, 8:00 AM – 5:00 PM) Pay Rate: $27 – $30 per hour Employment Type: Full-Time Overview We are seeking a skilled and compassionate Registered Dietitian to join our Chronic Care Management team. This is a remote, work-from-home role designed to provide ongoing dietary counseling and support to patients using weight management medications. The ideal candidate will excel in virtual patient engagement , using data from remote monitoring systems to guide personalized nutrition plans and conduct telehealth coaching sessions. Responsibilities Remote Patient Monitoring: Track and interpret patient scale readings and other relevant health data. Virtual Coaching: Conduct engaging one-on-one coaching sessions, providing personalized dietary education and motivation. Customized Diet Planning: Develop tailored meal plans for patients, particularly those on medications such as Ozempic. Patient Education: Educate patients on healthy eating habits, nutrition, and lifestyle changes to support chronic care goals. Collaboration: Work closely with physicians, pharmacists, and healthcare teams to ensure coordinated patient care. Documentation: Maintain accurate records of patient interactions, progress, and dietary plans using EMR platforms. Requirements Active Registered Dietitian (RD) license Experience in chronic conditions , weight loss , or related fields Proficient with remote monitoring platforms and/or EMR systems Strong communication skills for engaging patients virtually Must live and work full-time in the United States Reliable system setup: Windows Pro (i5 processor or newer) Nice to Have Experience in the Medicare CCM (Chronic Care Management) program Background in Remote Patient Monitoring (RPM) Bilingual (Spanish/English) is a plus Benefits Paid Time Off (PTO increases with tenure — 11 days in Year 1, 16 in Year 2) 401(k) Retirement Plan Health Care, Dental, and Vision Coverage No Weekend Work!
